Pre-Certification:
Pre-Certification is required for all hospital inpatient admissions and some out-patient procedures. Call TCC of SC at 1-888-275-7146. Emergency admissions must be certified within 72 hours.
When traveling or attending school out-of-area:
Call PHCS at 1-888-304-7427 for participating providers.

To File a STD or LTD claim by phone:

There will be further steps to be completed once the initial phone contact has been made.
Your doctor will need to provide an attending physicians statement to complete the process
for approval. The call center may fax the forms to your physician. Have your doctors contact information ready to provide to the call center. You may also obtain an attending physician statement by visiting your benefits coordinator located in the Detyens Medical Center building.
